Triad Prep Pad Recall (FSCA11-02)
There has been a voluntary recall of all Triad sterile and non-sterile alcohol Prep Pads.
The sterile version of the pad is included in PleurX Drainage kits with product codes 50-7500B and 50-7510. A Field Safety Notice has been issued requesting that all customers be formally advised not to use the Triad alcohol Prep Pads contained within the kits. Please see below for further details.
It is really important to note that the CareFusion products within the kits (i.e. the bottles and procedure packs) are perfectly safe to use. They are not affected by this recall and should be used as normal.
The Field Safety Notice has been posted to customers who have ordered these products. An advisory letter has been placed in each box and a warning label attached to each affected kit.
All affected products issued by UK Medical after 23 February 2011 now contain replacement pads.

The MHRA Alert relating to the above Field Safety Notice was formally closed on 1 June 2011 (MDA/2011/023 refers). Replacement pads will continue to be provided for all affected products.
